All of our physicians are now employed by Kootenai Clinic and their offices have moved to Kootenai Clinic Gastroenterology.  Patients being seen for an office visit or consultation (and not just for a procedure), should use the contact information listed here.

1. •If you are being seen in the office for the first time, or if there have been significant changes in your health history since the last time you were seen, please download and fill out the following form:

6. •All patients will need to present photo identification (preferably a driver’s license) as well as insurance information at the time of their visit.  Be prepared to pay any copay required by your insurance.
